An assessment is undertaken by an Occupational Therapist to determine an individuals capacity for independence in their day to day tasks within their home. A report is developed to provide recommendations and individual requirements both in the home and to access the community and is submitted to the referrer so they can consider providing the recommended funding.

Our staff are certified to use assessment tools such as the Functional Impairment Measure/WeeFIM and the Functional Assessment Measure.  These tools determine an individual’s community based capacity in a range of functional areas such as:

  • Self Care
  • Transfers
  • Mobility (internal and community)
  • Communication
  • Social Cognition
  • Leisure
  • Meal Preparation/Housekeeping
  • Budgeting
